2. The information to be sent to the Commission is divided into the following 2 separate components:

(1)  the information provided for in section 3;

(2)  the information provided for in section 4 that identifies the physician who administered medical aid in dying and the physician who gave a second opinion under subparagraph 3 of the first paragraph of section 29 of the Act respecting end-of-life care (chapter S-32.0001), as well as information that allows them to identify the person who requested medical aid in dying.
